About The Position

Silver Lake Hospital is a long-term acute care hospital with a reputation of providing patients with a place to heal before heading home. A long-term acute care hospital (LTACH) is an acute care specialty hospital that provides highly advanced and focused care for higher acuity patients who have not responded to short-term treatment in traditional general hospitals. The average length of stay in an LTACH is 25 days as compared to a 3-5 day average stay in a general hospital. Our safe and state of the art hospital is designed to admit and care for the most critically ill patients.

Requirements

  • Current NJ state LPN license.
  • At least (1) year experience as a Licensed Practical Nurse.
  • Basic Life Support (BLS) certification upon hire from the American Red Cross or the American Heart Association.
  • Advanced Cardio Life Support (ACLS) from American Red Cross or American Heart Association within two months of hire.
  • Age specific experience and/or special training and/or expertise is required to serve this population.

Nice To Haves

  • LTACH or ICU experience
  • Proficiency in EMR systems

Responsibilities

  • Under the supervision of the Registered Nurse (RN), the LPN provides direct and individualized nursing care to patients and implements plan of care formulated by physician and Registered Nurse.
  • Working under the RN, the LPN provides medication administration, ostomy care, wound care, oral care, draws lab work, and trach care.
  • Provides accurate and timely documentation consistent with plan of care.
  • Assesses and provides patient/client and family caregiver education and information pertinent to diagnosis and plan of care.
  • Maintains safe and healthy working environment and uses safe work methods and procedures in accordance with company clinical standards.
  • Participates in coordination with Registered Nurse and clinical management.
  • Assists patient in learning appropriate self-care activities.
  • The position involves direct patient care for a population of patients between the ages of 18 and older.
