The Licensed Practical Nurse (LPN) will be responsible for completing the admission process for consumers, which includes nursing assessments, medical checklists, physical and nutritional screenings, medication administration, and documentation. The LPN will observe and interact with consumers, providing direct nursing care and ensuring effective shift-to-shift communication. A key part of the role involves administering medications, educating consumers and families about them, and monitoring for side effects. The LPN will also interface with the multi-disciplinary treatment team, participate in treatment plan development, manage laboratory results, and provide emergency care as needed. The position requires adherence to professional ethics, continuous skill enhancement, and timely, accurate documentation. The LPN must treat consumers with dignity and compassion, respecting their privacy and confidentiality. They will also be responsible for maintaining communication through memos and emails, working independently or with teams, and supporting organizational policies regarding conduct, compliance, HIPAA, and security. Maintaining confidentiality of all consumer and organizational information is crucial. Adherence to attendance and punctuality policies, as well as safety and health standards (including TB tests, CPR/First Aid, CPI), is required. The role also involves identifying opportunities for service improvement and performing other assigned duties.
