About The Position

Fox Subacute South Philadelphia is a 53-bed skilled nursing and subacute facility specializing in the care of clinically complex respiratory and ventilator-dependent residents. Our environment offers the acuity level of a hospital step-down unit while allowing nurses to build long-term relationships with residents and deliver truly personalized care.

Requirements

• Active Pennsylvania LPN license (required)
• Current BLS/CPR certification
• Ability to work 12-hour shifts, including nights, weekends, and holidays as needed
• Strong communication and critical thinking skills
• Ability to remain calm during emergencies and high-acuity situations
• Ability to perform physical job duties, including lifting up to 50 pounds
• English proficiency preferred
• Ability to commute or relocate to Philadelphia, PA 19145

Responsibilities

• Assess, plan, implement, evaluate, and modify resident care plans
• Administer medications and treatments accurately and safely
• Respond to and manage emergency situations professionally and calmly
• Document care and clinical changes using PointClickCare
• Collaborate with the interdisciplinary healthcare team
• Deliver compassionate, individualized care aligned with resident needs
