About The Position
Our Lady of Mercy Life Center (OLOM) is seeking a Full-Time Evening Licensed Practical Nurse (LPN) for its Sub-Acute Rehabilitation unit. This role involves providing skilled nursing care to residents recovering from illness, injury, or surgery. The unit utilizes innovative technologies such as Smart Cell Flooring and an Artificial Sky. OLOM is part of St. Peter's Health Partners and is known for its exceptional outcomes, resident satisfaction, and dedicated staff. The LPN will administer medications and treatments, conduct resident assessments, assist in care plan development, educate residents and families, and collaborate with the interdisciplinary team.
Requirements
- Current New York State Licensed Practical Nurse (LPN) license.
- Graduate of an accredited Practical Nursing program.
- Strong clinical assessment and communication skills.
- Compassion, teamwork, and a commitment to providing exceptional resident-centered care.
Nice To Haves
- Long-term care, rehabilitation, or skilled nursing experience is preferred, but new graduates are encouraged to apply.
Responsibilities
- Provide skilled nursing care to residents in our Sub-Acute Rehabilitation unit.
- Administer medications and treatments in accordance with physician orders and nursing standards.
- Conduct resident assessments and report changes in condition to the RN Supervisor and healthcare team.
- Assist in developing, implementing, and updating individualized care plans.
- Educate residents and families about treatments, recovery goals, and ongoing care needs.
- Collaborate with interdisciplinary team members to support quality outcomes and successful rehabilitation.
