About The Position

Mercy Rehabilitation Hospital is hiring a full-time Rehabilitation Licensed Practical Nurse to provide patient-centered care on a night schedule of three 12-hour shifts, 7:00 PM to 7:00 AM. The hospital’s rehabilitation team helps patients regain independence and improve their quality of life following serious illness, injury, or complex medical conditions. The LPN works collaboratively with the RN and interdisciplinary team to support patient recovery in a compassionate, safe, and supportive environment.

Requirements

• Current state or compact-state LPN license; current state licensure as a Licensed Practical/Vocational Nurse.
• AHA Basic Life Support (BLS) certification.
• Graduate of an accredited Licensed Practical/Vocational Nurse program.

Nice To Haves

• At least six months of Medical/Surgical experience in an acute care setting is preferred.

Responsibilities

• Assist the RN with initial assessments and patient reassessments according to policy.
• Identify and implement appropriate rehabilitation nursing interventions.
• Complete timely and accurate Quality Improvement (QI) assessments under RN supervision.
• Deliver care based on patient response to treatment and update the interdisciplinary plan accordingly.
• Assess and reassess pain, apply appropriate pain management techniques, and educate patients and families.
• Perform patient care tasks in a manner that promotes patient safety and reduces medical errors.
• Develop and implement individualized teaching plans based on patient and family learning needs.
• Perform other duties as assigned.
