About The Position

Paradise Creek Health and Rehabilitation of Cascadia is looking for dedicated and skilled Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to join our team! Requirements

Current LPN or RN license in the state
Strong clinical and critical thinking skills
Excellent communication and interpersonal abilities
Ability to work effectively as part of a team
Compassionate and patient-centered approach to care
Ability to manage multiple tasks and prioritize effectively
Graduate of accredited LPN School of Nursing.
Current valid LPN license in the state employed.
Valid CPR certification.

Nice To Haves

Previous experience in long-term care, geriatric, or rehabilitation settings is a plus
Six months experience in a long-term care environment preferred.

Responsibilities

Administer medications and treatments as prescribed
Monitor residents' health status and report any changes to the healthcare team
Develop and implement individualized care plans in collaboration with residents, families, and the interdisciplinary team
Supervise and support Certified Nursing Assistants (CNAs) in delivering resident care
Assist with admission, discharge, and transfer processes
Ensure accurate and timely documentation in accordance with state and federal regulations
Provide emotional support to residents and their families
Participate in ongoing education and training to maintain clinical competence
