Requirements

Current LPN License in State of operations.
LPN Diploma & State License Required
Knowledge of drug and alcohol treatment preferred.
Working knowledge of Google Products
Current LPN License
BLS
AED

Responsibilities

Assessment and evaluation of patient withdrawal followed by the execution of appropriate medication detox protocols.
Administration of prescribed medications and patient education.
Provide patients with a safe medically managed detox by assessing signs and symptoms of withdrawal.
Monitor the effects of medications administered and document accurately and appropriately.
Report and document medication errors, pharmacy errors, packaging errors, medication count concerns, etc.
Ensure ongoing assessments are conducted as determined by the patient’s condition using COWS & CIWAS and re-prioritize care accordingly.
Maintain neat and accurate narcotic control inventories and all documentation related to the administration of medications.
Promote patient's independence by establishing patient care goals, teaching patient to understand condition, medications, and self-care requirements, and answering patient questions.
Assure quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following philosophies and standards of care set by State Board of Nursing, and other governing agency regulations.
Resolve patient problems and needs by utilizing multidisciplinary team strategies, including de-escalation techniques.
Document patient care services by charting in EMR.
Maintain continuity among teams by documenting and communicating actions, irregularities, and continuing needs.
Contribute to team effort by accomplishing related results as needed.
Participate in the onboarding of new nurses for medication administration.
At end of shift communicate to incoming LPN pertinent data regarding patient activity and conduct medication count.
Treat patients and colleagues with dignity and respect; honor each person’s right to privacy/maintain very strict patient and colleague confidentiality; respect all individual differences; support cultural diversity.
Know role and respond appropriately to all codes related to emergency situations.
Consistently arrive to work on time and adhere to attendance policies.
