AdCare Hospital is seeking a Licensed Practical Nurse (LPN) to join our team on a PRN basis. Requirements

  • Current LPN License in State of operations.
  • LPN Diploma & State License Required
  • Current LPN License
  • BLS
  • AED

Nice To Haves

  • Knowledge of drug and alcohol treatment preferred.
  • Working knowledge of Google Products

Responsibilities

  • Assessment and evaluation of patient withdrawal, followed by the execution of appropriate medication detox protocols.
  • Administration of prescribed medications and patient education.
  • Provide patients with a safe medically managed detox by assessing signs and symptoms of withdrawal.
  • Monitor the effects of medications administered and document accurately and appropriately.
  • Report and document medication errors, pharmacy errors, packaging errors, medication count concerns, etc.
  • Conduct ongoing assessments as determined by the patient’s condition using COWS & CIWAS and re-prioritize care accordingly.
  • Maintain neat and accurate narcotic control inventories and all documentation related to the administration of medications.
  • Promote patient's independence by establishing patient care goals, teaching patients to understand their condition, medications, and self-care requirements, and answering patient questions.
  • Assure quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following philosophies and standards of care set by State Board of Nursing, and other governing agency regulations.
  • Resolve patient problems and needs by utilizing multidisciplinary team strategies, including de-escalation techniques.
  • Document patient care services by charting in EMR.
  • Maintain continuity among teams by documenting and communicating actions, irregularities, and continuing needs.
  • Contribute to team effort by accomplishing related results as needed.
  • Participate in the onboarding of new nurses for medication administration.
  • At end of shift, communicate to incoming LPN pertinent data regarding patient activity and conduct medication count.
  • Treat patients and colleagues with dignity and respect; honor each person’s right to privacy/maintain very strict patient and colleague confidentiality; respect all individual differences; support cultural diversity.
  • Know role and respond appropriately to all codes related to emergency situations.
  • Consistently arrive to work on time and adhere to attendance policies.
  • Other duties, as needed.
