About The Position

RPM Healthcare is seeking a Licensed Practical Nurse (LPN/LVN) with a Compact/Multistate License to join their team as an RPM 365 Care Coach. This role involves engaging patients, tracking and analyzing their physiological data (Blood Pressure, Weight, Blood Glucose, Lung Capacity, Oxygenation), providing education, and encouraging adherence to care plans. The company has nearly 40 years of patient education experience and utilizes proprietary software to identify dangerous out-of-range values and trends that impact health. The ideal candidate is passionate about healthcare innovation and patient outcomes, tech-savvy, comfortable with high-volume phone outreach (125+ conversations a week), and thrives in a fast-changing environment. They should be patient, empathetic, detail-oriented, and a self-starter.

Requirements

• Active nursing license that is a compact/multistate.
• Knowledge of chronic conditions and related monitoring devices (weight management a plus).
• Proficiency with technology, computer platforms, and smartphones.
• Strong verbal/written communication skills.
• Comfortable with patient outreach and engagement.
• Highly organized, detail-oriented, and team-focused.
• Independently motivated.
• Comfortable spending the majority of your workday engaging with patients over the phone providing support, education and guidance.

Nice To Haves

• Experience in telehealth, patient monitoring, education, or customer service.
• Additional language skills are a plus.

Responsibilities

• Serve as the primary contact for patients on the RPM365 platform.
• Assist with device setup and app connectivity.
• Engage with patients to ensure adherence to care plans.
• Schedule monthly check-ins and monitor patient data.
• Educate patients through shared resources.
• Notify healthcare providers of critical alerts.
• Attend virtual team meetings for updates.
