About The Position

We are seeking a dedicated Licensed Practical Nurse (LPN / LVN) for a 13-week contract position in Utica, NY. This role involves providing direct patient care in a long-term care setting, administering medications and treatments, and collaborating with a healthcare team. The position requires flexibility to work 8-hour night shifts, including every other weekend, and the ability to float between units as needed. The LPN will care for patients in a 40-bed unit, supporting various care needs including medical-surgical, orthopedic, dementia, and memory care.

Requirements

• Minimum 1+ year of recent experience as an LPN in long-term care – Required.
• Strong communication, observation, and clinical skills.
• Ability to work independently, multitask, and float between units.
• Comfortable with weekend and night shift rotations.
• Graduate of an accredited Licensed Practical Nursing (LPN) program – Required.
• LPN License – New York State – Required.
• Basic Life Support (BLS – AHA) – Required.

Nice To Haves

• Experience with EMR systems, especially PointClick Care – Preferred.

Responsibilities

• Provide direct patient care under the supervision of a registered nurse or physician in a long-term care setting.
• Administer medications and treatments as prescribed and document accurately in the EMR (PointClick Care).
• Assist with daily living activities including bathing, dressing, feeding, and toileting.
• Monitor patient conditions and report any changes to the supervising RN or physician.
• Float to all units within the same building as needed to support care continuity.
• Care for a 40-bed unit population including medical-surgical, orthopedic, dementia, and memory care patients.
• Maintain accurate records of patient care, condition, progress, and concerns.
• Ensure adherence to safety and infection control policies at all times.
• Collaborate with interdisciplinary teams to deliver holistic and person-centered care.
