The Licensed Practical Nurse (LPN) provides skilled nursing care under the direct supervision of the Registered Nurse. Performs selected skilled actions in the provision of curative, rehabilitative, palliative, or preventative nursing care. Is responsible and accountable for making decisions that are based on the individual's nursing experience and educational preparation. The LPN assists the Registered Nurse in carrying out the Plan of Care and in performing specialized procedures as directed by the Registered Nurse under physician's orders. They also assist with the preparation, implementation, and on-going evaluation of the client care plan. The role involves recognizing and understanding the effects of social and economic problems upon clients and providing for their emotional and physical comfort and safety. The LPN helps teach patients appropriate self-care techniques and fosters cooperative effort among personnel by understanding the functions of other persons involved in client care and by actively participating in team and staff conferences. They are expected to assume responsibility for personal and vocational growth and development through active participation in nursing organizations and in-service programs and other on-the-job learning. The LPN enters clinical/progress notes into EMR within 24 hours of a scheduled visit. They observe, record, and report to the appropriate person the patient’s symptoms, reactions, and changes, including general physical and mental condition, signs and symptoms indicative of untoward changes, stresses in human relationships, and any abnormality in the client’s range of motion, body mechanics, or body alignment. Maintaining current TB Testing is required, and the LPN must be knowledgeable of the Agency’s organization, functions, and services, and comply with the Agency dress code policy. Efficient flow of Agency work is maintained by reporting to work at assigned or scheduled times and following procedures for notification of absence or time of requests. Advanced LPNs with documented skills and specific agency/state approval may perform certain treatments/procedures such as administering medications, hanging hydrating fluids without medications, flushing peripheral lines with saline or heparin solutions, calculating and adjusting flow rates, hanging pre-mixed IVs, starting peripheral IVs, inspecting insertion sites, changing dressings, removing peripheral IV needles/catheters, and checking blood with a Registered Nurse.
