Requirements

Current, unrestricted New York State Licensed Practical Nurse (LPN) license.
Current BLS/CPR Certification.
Excellent communication, organizational, and critical thinking skills.
Ability to work independently and as part of a multidisciplinary team.
Reliable transportation.

Nice To Haves

Minimum of one (1) year of LPN experience preferred.
Experience working with individuals with intellectual and developmental disabilities is a plus, but not required.

Responsibilities

Provide direct nursing care under the supervision of a Registered Nurse or physician.
Administer medications and treatments as prescribed.
Monitor and document vital signs, health status, and changes in patient condition.
Assist with developing and implementing individualized care plans.
Maintain accurate and timely patient documentation.
Provide health education and support to individuals and direct care staff.
Assist individuals with medical needs while promoting dignity, independence, and quality of life.
Ensure compliance with agency policies, New York State regulations, and HIPAA standards.
Respond appropriately to medical emergencies and communicate effectively with the interdisciplinary healthcare team.
Maintain a safe, clean, and therapeutic environment.
