Licensed Practical Nurse (LPN) – Home Health | Grants Pass, OR
About The Position
This is a direct hire opportunity in Grants Pass, Oregon, offering the kind of full-scope clinical work and schedule stability that home health LPNs consistently say they're looking for. The role offers a rare Monday through Friday schedule in home health nursing, allowing for a consistent routine while providing one-on-one, in-home patient care. You will provide wound care, medication administration, injections, IV therapy, and vital signs monitoring, utilizing the full range of your LPN skill set. You will also build real patient relationships by working in their homes and have a voice on the care team by collaborating with the RN Case Manager and interdisciplinary team on care planning, and providing oversight and feedback to Home Health Aides.
Requirements
- Graduate of an accredited practical or vocational nursing program
- Current, active LPN or LVN license in the state of Oregon
- One year of clinical experience, including one year specifically in home health
- Valid CPR certification, maintained throughout employment
- Valid driver's license and reliable transportation to travel to patient homes across the assigned territory
Responsibilities
- Travel within your assigned territory to deliver skilled nursing visits per each patient's physician-ordered plan of care, under RN supervision
- Perform hands-on treatments: wound care, medication administration, injections, IV therapy, and vital signs checks
- Monitor each patient's condition and report changes promptly to the RN or physician
- Educate patients and families on disease management, medications, safety, and self-care so they can manage more independently between visits
- Supervise Home Health Aides on assigned care tasks
- Complete clinical documentation accurately and on time, in line with agency, state, and payer requirements
- Join interdisciplinary case conferences and quality improvement discussions
- Follow strict safety and infection control protocols on every visit
