Requirements

Active Minnesota Licensed Practical Nurse (LPN) license.
Reliable transportation with a valid driver's license and current insurance.
Ability to work independently and manage your daily schedule.
Strong communication skills and a passion for patient-centered care.

Nice To Haves

Previous nursing experience in skilled nursing, rehab, TCU, hospice, or home health is preferred.
Experience as an Army 68C , Navy Hospital Corpsman (HM) , Air Force 4N0X1 , Marine Corps HM , or Coast Guard Health Services Technician (HS) is highly valued.

Responsibilities

Provide skilled nursing visits in patients' homes.
Perform wound care, medication administration, and disease management.
Reinforce education provided by the RN Case Manager.
Monitor patient conditions and communicate changes to the care team.
Collaborate with physicians, therapists, and fellow clinicians.
Complete accurate and timely documentation.
