About The Position

Home Health Care, Inc. is seeking Licensed Practical Nurses (LPNs) for home care visits in the Twin Cities Metro Area. This role offers a different way to practice nursing, focusing on one-on-one patient interactions, independence, and a predictable, flexible schedule. Unlike facility-based roles, this position allows nurses to control their day, prioritize quality of care, and see the long-term impact of their work. Requirements

Active Minnesota Licensed Practical Nurse (LPN) license.
Reliable transportation, valid driver's license, and current auto insurance.
Ability to work independently and manage a daily field schedule.

Nice To Haves

Previous experience in skilled nursing, rehabilitation, TCU, hospice, or home health is helpful but not required.
Compassionate clinicians who enjoy building relationships with patients.

Responsibilities

Perform skilled nursing visits in patients' homes.
Provide wound care, medication management, and ongoing patient assessments.
Educate patients and caregivers to promote independence.
Communicate changes in condition with the RN Case Manager and healthcare team.
Complete accurate and timely documentation.
Help patients remain safe, healthy, and comfortable at home.
