Requirements
- Current LPN license in the state of Michigan
- Strong organizational and time-management skills
- Excellent communication and interpersonal skills
- Compassionate, reliable, and detail-oriented
Nice To Haves
- Experience with tube feeding and in-home care preferred
Responsibilities
- Provide skilled nursing care in accordance with the care plan
- Perform tube feedings and monitor nutritional intake
- Assist with and guide range of motion exercises
- Monitor and report changes in client condition
- Maintain accurate and timely documentation
- Coordinate and manage doctor appointments and transportation logistics
- Support scheduling and participation in social and recreational activities
- Communicate effectively with family members and the care team
