The Licensed Practical Nurse (LPN) – Home Care position is a full-time role based with the Millersburg team, serving the southern portion of the region. This role requires extensive driving throughout the workday and participation in a weekend rotation, along with two holiday shifts per year. Mileage reimbursement is provided. Requirements

  • Current Ohio Licensed Practical Nurse (LPN) license in good standing
  • Current CPR certification
  • Basic computer skills for electronic documentation
  • Valid driver's license with a safe driving record
  • State minimum automobile insurance coverage
  • Ability to travel extensively throughout the service area, including daytime, evening, and inclement weather driving

Nice To Haves

  • One year of nursing experience preferred

Responsibilities

  • Provide skilled nursing care under the direction and supervision of a Registered Nurse (RN) in accordance with the patient's plan of care and within the LPN scope of practice.
  • Assist with nursing assessments by observing, collecting, and reporting patient information to the RN.
  • Deliver care that addresses the physical, emotional, spiritual, and psychosocial needs of patients and their families.
  • Monitor patient condition, recognize changes in status, and promptly communicate findings to the RN.
  • Administer medications as prescribed using the correct dose, route, and frequency.
  • Educate patients and caregivers on self-care techniques as directed by the RN.
  • Document patient care, observations, and interventions accurately and promptly in the electronic health record.
  • Collaborate with the interdisciplinary team to promote patient comfort, safety, and quality outcomes.
  • Assist other departments and team members as needed to ensure comprehensive patient care.
  • Participate in Continuous Quality Improvement initiatives.
  • Serve as a preceptor for new team members when assigned.
