About The Position

Merakey is seeking a compassionate Licensed Practical Nurse (LPN) to join their Assertive Community Treatment (ACT/FACT) team. The LPN will play a key role in supporting individuals with serious mental illness through a recovery-focused approach. This involves providing clinical services, developing treatment plans, conducting assessments, supporting health needs including medication management, and acting as a liaison with families and community agencies.

Requirements

  • Licensed Practical Nurse (LPN). Active LPN license issued by the Virginia Board of Nursing
  • 2 years of experience providing mental health services to adults with serious mental illness
  • At least 1 year of experience in medication management
  • Strong knowledge of recovery principles and recovery-driven care
  • Valid Virginia driver’s license

Responsibilities

  • Provide recovery-based, person-centered clinical services through individual and group therapy, supporting mental health, daily living skills, and social development.
  • Develop, implement, and regularly update treatment plans in collaboration with individuals, families, and the interdisciplinary team.
  • Conduct comprehensive assessments and provide ongoing evaluation of symptoms, treatment responses, and overall progress.
  • Support overall health needs by providing medication education, monitoring, administration, and addressing physical health concerns.
  • Act as a liaison and advocate by coordinating with families and community agencies, offering education, crisis support, and connections to resources.
