About The Position
Under the guidance of the Resident Care Manager and in close collaboration with a diverse healthcare team, the Licensed Practical Nurse (LPN) delivers top-notch nursing care to long-term care residents in addition to providing emotional comfort and support to residents and their families. LPNs in long-term care have the unique opportunity to build deep and lasting relationships with residents/families/caregivers, collaborate with a compassionate and knowledgeable interdisciplinary care team, and contribute to an overall sense of "home" for residents. Chenchenstway, meaning "lifting each other up", is a 3-storey, 110 bed long term care home located in Burnaby. It is a joint operation between Providence Health Care and Fraser Health, attracting staff who are open-minded, adaptable, and who thrive in challenging situations where leadership and innovative problem-solving is required. Requirements
- Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training and experience
- Registration with the British Columbia College of Nurses and Midwives as a practicing LPN registrant
- Current basic cardiac life support certification
- Demonstrated knowledge and ability to perform Cardiopulmonary Resuscitation (CPR) procedures appropriate for the area of service
- Physical ability to carry out the duties of the position
- Ability to apply decision making and analytical skills
- Ability to role model conflict resolution strategies
- Ability to prioritize and organize work
- Ability to teach and identify patient/resident learning needs
Responsibilities
- Provide safe and respectful nursing care, adhering to established policies, individualized care plans, and priorities
- Conduct thorough assessments, monitor signs/symptoms, and communicate with the RN/RPN as needed
- Prepare patients for procedures, transfers, and admissions, coordinating with the interdisciplinary team
- Administer medications per protocol, monitor effects, and maintain communication with the team
- Utilize evidence-based resources to stay updated on nursing practices
- Respond to incidents such as codes, injuries, spills, and alarms, following procedures
- Assist with staff orientation, demonstrating procedures and providing support
- Address patient/resident and family learning needs, provide educational materials, and make referrals
- Attend care planning meetings, provide input, and advocate for patients/residents
- Collaborate with the team to develop and evaluate individualized care plans
- Assist with discharge planning based on patient status and external supports
- Operate and clean equipment, report malfunctions, and handle hazardous materials.
- Perform clerical duties such as answering phones, faxing, and maintaining records
