This role involves providing safe and respectful nursing care to an assigned group of patients/residents in a long-term care setting. The Licensed Practical Nurse (LPN) will adhere to established policies, individualized care plans, and priorities. Responsibilities include conducting thorough assessments, monitoring patient signs and symptoms, administering medications, collaborating with the interdisciplinary team, and assisting with patient care activities such as procedures, transfers, admissions, and discharge planning. The LPN will also be involved in patient/resident education, responding to incidents, assisting with staff orientation, and maintaining accurate documentation. The position operates within the framework of minimum nurse-to-patient ratios (mNPR) and upholds the organization's mission and values.
