About The Position

The Mental Health Transitional Living Homes Program, established by House Bill (HB) 22-1303, aims to increase residential behavioral health treatment capacity for adults in Colorado. This program provides a step-down option from inpatient care and a step-up for individuals needing a higher level of care than lower-level settings can safely provide. The Nurse I position is crucial for executing daily clinical operations within these homes, focusing on medication administration, clinical assessments, order transcriptions, and health education. This role is integral to ensuring client safety, supporting treatment continuity, and promoting client independence as part of a multidisciplinary team.

Requirements

Current, valid licensure as a Licensed Practical Nurse from the Colorado Board of Nursing or any state participating in the Enhanced Nurse Licensure Compact (eNLC).
Completion of a Board-approved IV certification course including supervised clinical, or documentation of completion from another state.
Maintenance of evidence of original documentation and demonstration of IV knowledge/judgment/skill.
Adherence to written IV policies and procedures approved by medical staff, nursing, and agency administration.
Experience in hospital settings (including mental health), Department of Corrections, Detox/Rehab Facilities, and medical environments.

Nice To Haves

Experience working with adults diagnosed with Serious Mental Illness (SMI).
Qualified in electronic medication administration records (eMAR).
Clinical and psychiatric assessment skills.
Knowledge of psychotropic medications and medication monitoring.
Experience providing medication education to clients.
Experience facilitating psychoeducational or nursing groups.
Verbal de-escalation and crisis intervention skills.
Knowledge of trauma-informed, recovery-oriented, and person-centered care.
Experience working within a multidisciplinary treatment team.

Responsibilities

Administer Long-Acting Injectables (LAIs) and routine medications according to the Medication Administration Record (MAR).
Accurately transcribe written physician orders and update MARs/TARs (Treatment Administration Records).
Conduct regular medication counts and coordinate with the pharmacy for reordering and inventory management.
Perform physical and psychiatric assessments to evaluate client status, including vital signs and basic mental status checks.
Document all client findings accurately and chronologically within the Electronic Health Record (EHR).
Prepare and facilitate client groups focused on health education, medication management, personal hygiene, and infection control.
Collaborate with the Advanced Practice Provider, MHTL Home clinical team, and Medical Director to optimize client care.
Facilitate safe and structured client admissions and discharges as part of the multidisciplinary treatment team.
Attend and contribute clinical insights during Individualized Service Plan (ISP) meetings.
Perform other duties as assigned.
