About The Position

The Licensed Practical Nurse I – Residential delivers quality health care services to youth in a residential setting under the direction and supervision of a Registered Nurse (RN). The role includes case finding, health teaching and counseling, and supportive and restorative care. The position is based at the Monroe Campus in Rochester, New York.

The schedule is 3:00 p.m. to 11:30 p.m., 40 hours per week, Tuesday through Saturday. An alternative schedule may be discussed. The employer is not seeking per-diem staff for this position. The posting offers a sign-on bonus of up to $4,000 for full- or part-time LPNs and shift differentials of up to $6.50 per hour for eligible evening, overnight, and weekend hours. Pay is $24.00 to $33.00 per hour, based on experience.

Requirements

• Successful completion of an accredited LPN program.
• At least 1 year of related work experience.
• Active, unrestricted New York State LPN license.
• Basic Life Support (BLS) training within 60 days of hire and annual recertification.
• Ability to recognize health needs of children and youth and communicate effectively with youth and families.
• Accurate knowledge of applicable regulatory and practice requirements, treatment protocols, and HIPAA requirements.
• Ethical and professional conduct.
• Proficiency with computer-based applications and data entry or use.
• Knowledge of agency and external provider services and ability to maintain contacts to access client care.
• Ability to work a variable schedule that includes evenings and weekends.
• Ability to meet the described physical demands, including lifting and carrying items weighing up to 50 and 55 pounds, respectively.
• Valid, unrestricted New York State driver's license held for at least 1 year, clean driving record, and minimum insurance coverage meeting agency standards preferred.

Responsibilities

• Administer medications safely and accurately in accordance with medication policy, and promptly report unusual medication reactions to the RN.
• Document progress notes clearly, accurately, and promptly in the electronic medical record and other required systems, following professional and agency standards.
• Order and maintain inventory of stock medications and medical supplies.
• Provide basic first aid and CPR in emergency situations.
• Provide ongoing medication, wellness, and health education to youth.
• Facilitate and assist with scheduling youth appointments as directed.
• Take responsibility for work commitments and actions, and recognize and correct errors or omissions.
• Follow applicable safety regulations, including universal precautions and hazardous-material safety rules.
• Support process-improvement initiatives in collaboration with leadership.
• Follow applicable federal, state, and local statutes, regulatory standards, and Hillside policies, including standards and requirements of OMH, OCFS, COA, and NYSED.
• Perform other duties as assigned.
• Recognize current and emerging health needs of children and youth, address issues proactively, and communicate effectively with youth and families about treatment, issues, and objectives.
• Maintain accurate knowledge of regulatory and practice requirements and treatment protocols, with particular attention to HIPAA.
• Maintain high standards of ethical and professional conduct toward youth and families.
• Use computer-based applications to independently complete work and enter or use data.
• Maintain knowledge of agency and external provider services and contacts to efficiently access care for clients.
