About The Position

Sentara Health is seeking to hire a qualified individual to join our team as a Licensed Practical Nurse. The Licensed Practical Nurse (LPN) in a home health care setting is responsible for providing direct, one-on-one care to patients in their homes. This role involves monitoring patient health, administering treatments, managing medications, and offering essential support to patients with various medical needs. Collaboration with a team of healthcare professionals, including Registered Nurses (RNs) and physicians, is required to deliver high-quality, compassionate care that improves patient well-being.

Requirements

• LPN License in state of practice (VA)
• BLS (Required within 90 days)
• 2 years' experience

Responsibilities

• Providing direct, one-on-one care to patients in their homes.
• Monitoring patient health.
• Administering treatments.
• Managing medications.
• Offering essential support to patients with various medical needs.
• Collaboration with a team of healthcare professionals, including Registered Nurses (RNs) and physicians, to deliver high-quality, compassionate care that improves patient well-being.
• Serve as patient advocate in the prevention of illness or disease.
• Provide care and treatment for patients and clinical support and teaching for patient and family members.
• Customer service, testing, medication administration and the implementation of nursing interventions and care plans.
• Support a holistic approach to patient care.
