About The Position

Concordia at Bethlen is a faith-based, non-profit continuing care retirement community located in historic Ligonier, Pennsylvania, which joined the Concordia family in 2023. Bethlen has a rich history of caring for others since 1921, offering 44 retirement living cottages, 72 personal care units, and 96 skilled nursing units. Concordia Home Health and Hospice of Bethlen is seeking a dedicated and personable Hospice LPN to provide end-of-life care in and around the Scottdale area. This role offers the opportunity to make a true difference in patients' health and well-being, build deep relationships with patients and their families, and enjoy autonomy and flexibility in managing one's schedule. The position also provides continuous growth through paid training, scholarships, and opportunities to specialize skills, all within a supportive and collaborative team environment.

Requirements

Licensed Practical Nurse (LPN)
Strong clinical skills and knowledge of end-of-life care principles
Excellent communication, interpersonal, and organizational skills
Compassion, empathy, and emotional resilience to provide comfort and support during end-of-life situations
Ability to work independently and as part of a collaborative team

Responsibilities

Administering medications and treatments
Monitoring vital signs
Assisting with wound care and personal care
Educating patients and families
Maintaining medical records
Collaborating with other healthcare professionals
Provide emotional support and companionship, building genuine connections with patients
