About The Position

As a Licensed Practical Nurse on our team, you will provide skilled nursing care, support patients’ health needs, collaborate with the healthcare team, and offer compassionate guidance to patients and their families.

Requirements

• Graduate of an approved school of practical nursing, currently licensed as a Licensed Practical Nurse in the State of Indiana
• Car with required proof of insurance coverage and an Indiana Driver’s license
• Knowledge of all types of home health equipment utilized by patients as well as forms and paperwork necessary for documentation

Nice To Haves

• Two (2) years of experience in home health care or related health field

Responsibilities

• Provide safe, high-quality home health care.

• Communicate effectively with clients, families, and the care team.
• Educate clients, families, and staff as needed.
• Use equipment and supplies efficiently.
• Complete documentation accurately and on time.
