About The Position

As a Licensed Practical Nurse (LPN) in our Home Health program, you’ll serve as a trusted advocate and care partner for patients facing a wide range of health challenges. Working closely with Case Managers and interdisciplinary care teams, you’ll help patients understand their treatment options and support them in making informed decisions that align with their unique needs and goals. Requirements

  • Graduate of an accredited nursing program
  • Active state LPN license, CPR certification, valid driver’s license
  • Current physical exam and TB test
  • Strong clinical, communication, and problem-solving skills
  • Compassionate attitude toward caring for the sick and elderly
  • Ability to follow directions, read, and write

Nice To Haves

  • Previous home health/OASIS experience is a plus
  • Home health and OASIS experience preferred

Responsibilities

  • Deliver high-quality, patient-centered nursing care in home settings
  • Collaborate with Case Managers to assess patient needs and coordinate care plans
  • Educate patients and families about treatment options, medications, and self-care strategies
  • Serve as a liaison between patients, providers, and community resources
  • Monitor patient progress and report changes to the care team
