We are seeking a Licensed Practical Nurse (LPN) who is hands-on, detail-oriented, and passionate about patient care. In this role, you’ll work closely with an RN Case Manager to provide skilled nursing services that support patient safety, recovery, and independence.
Requirements
- Current, unrestricted Licensed Practical Nurse (LPN) license
- Minimum of 1 year of clinical nursing experience
- Strong communication and documentation skills
- Ability to work independently while staying connected to a care team
- Willingness to travel within assigned territory (approximately 50%)
- Valid driver’s license, reliable transportation, and current auto insurance
- Ability to lift/move up to 50 - 100 lbs and assist with patient mobility

Responsibilities
- Provide skilled nursing care under the direction of an RN and physician orders
- Review and follow the established Plan of Care during each patient visit
- Perform routine nursing visits and deliver treatments within scope of practice
- Monitor patient conditions and report changes to the RN Case Manager and care team
- Accurately document visits, care provided, and patient progress
- Assist with medication management and maintain up-to-date medication records
- Educate patients and caregivers on basic care, safety, and self-care techniques
- Identify and report safety concerns in the home environment
- Collaborate with the RN Case Manager to support care plan updates and patient goals
- Help prevent avoidable hospitalizations through proactive patient monitoring and care
