Telehealth responsibilities involve collecting information before provider visits, researching prescription discrepancies, and facilitating prior authorization denials. The LPN collaborates daily with various healthcare professionals and support staff. The licensed practical nurse (LPN) provides nursing care under the direct supervision of a registered nurse (RN) or other assigned supervisor, accepting delegation to meet patient/family needs. The LPN collects observational data and communicates findings to assist the RN/provider in patient assessment and care planning. The LPN is a team member who delivers care according to the patient care plan, organizational policies, and relationship-based care principles. The role requires excellent communication, mentoring, and instructional skills, with potential participation in committees or quality improvement projects.

Requirements

  • Graduate of a school of practical nursing or passed LPN Boards after a defined period in a professional nursing program
  • If graduation did not occur within the last two years, one year of LPN experience working in an applicable setting is required
  • Ability to adapt to unpredictable situations within the clinical and/or research settings
  • Able to work effectively with multiple members of the interdisciplinary care team
  • Demonstrates strong, effective and open communication skills to achieve mutual understanding and build trust and rapport with team members
  • Able to work in a rapidly changing environment while participating in developing innovative and creative solutions to patient care
  • Excels in creative experimentation and is able to explore new paths, procedures, and /or approaches to models of care teams
  • Displays flexibility in adapting and coping with accelerated changes within the environment
  • Current LPN license by applicable state requirements
  • BLS (Basic Life Support) must be valid for 90 days post start date; acceptable certifications include: American Heart Association (AHA)/ Heartcode® BLS, Military Training Network, Red Cross (CPR/AED for Professional Rescuers and Health Care Providers)
  • Additional state licensure(s) and/or specialty certification/training as required by the work area

  • Collect data through observation and communicate information to assist the RN and/or provider in patient assessment and care planning
  • Participate as a member of the health care team and accept delegation from the RN/assigned supervisor and provider in meeting needs of the patient/family
  • Deliver care in accordance with the patient care plan, policies and procedures of the organization and principles of relationship-based care
  • Possess excellent communication skills
  • Be skillful in mentoring and instructing
  • May participate on committees or projects, including quality improvement projects
