Requirements

  • Graduate of a school of practical nursing or passed LPN Boards after a defined period in a professional nursing program.
  • If graduation did not occur within the last two years, one year of LPN experience working in an applicable setting is required.
  • High school diploma or GED equivalent.
  • Maintains Basic Life Support (BLS) competency (Arizona).
  • Additional state licensure(s) and/or specialty certification/training as required by the work area.

Nice To Haves

  • Experience in ambulatory/outpatient setting preferred.
  • Family Medicine/Primary Care experience preferred.
  • Strong customer service skills for a high-touch patient population required.
  • Excellent verbal and written communication skills.
  • Ability to work effectively in a team environment.
  • Strong critical thinking and problem-solving abilities.
  • Ability to adapt to unpredictable situations within the work setting.
  • Computer proficiency required, including experience with electronic medical records.
  • Epic experience preferred.
  • Flexibility to meet staffing needs and work daytime hours.
  • Two years of recent LPN experience preferred.
  • Prior phlebotomy experience preferred.

Responsibilities

  • Provides nursing care to assigned patients under the direct supervision of the registered nurse (RN) or other assigned supervisor.
  • Accepts delegation from the RN/assigned supervisor in meeting the needs of the patient/family.
  • Collects data through observation and communicates information to assist the RN and/or provider in patient assessment and care planning.
  • Participates as a member of the health care team.
  • Delivers care in accordance with the patient care plan, policies and procedures of the organization and principles of relationship-based care.
  • Possesses excellent communication skills.
  • Is skillful in mentoring and instructing.
  • May participate on committees or projects, including quality improvement projects.
