Requirements

  • Graduate of a school of practical nursing or passed LPN Boards after a defined period in a professional nursing program.
  • If graduation did not occur within the last two years, one year of LPN experience working in an applicable setting is required.
  • Excellent communication skills (verbal and written).
  • Experience working in a team environment.
  • Computer skills required.
  • Ability to work daytime hours, flexibility may be required to meet staffing needs.
  • Ability to adapt to unpredictable situations within the work setting.
  • Current LPN license by applicable state requirements.
  • Maintains current Basic Life Support (BLS) for Health Care Providers (specific requirements vary by location: Arizona and Florida require BLS for Health Care Providers; Midwest requires BLS for Health Care Providers from American Heart Association or American Red Cross).
  • Basic Life Support (BLS) for Health Care Providers from American Heart Association or American Red Cross must be valid for 90 days prior to start date.

Nice To Haves

  • One year LPN experience within the last five years preferred.
  • Prior experience with electronic medical record systems preferred.
  • Graduate of an accredited school of practical nursing preferred.
  • Additional specialty certification/training as required by the work area.

Responsibilities

  • Provides nursing care to assigned patients under the direct supervision of the registered nurse (RN) or other assigned supervisor.
  • Accepts delegation from the RN/assigned supervisor in meeting the needs of the patient/family.
  • Collects data through observation and communicates information to assist the RN and/or provider in patient assessment and care planning.
  • Participates as a member of the health care team.
  • Delivers care in accordance with the patient care plan, policies and procedures of the organization and principles of relationship-based care.
  • Possesses excellent communication skills; is skillful in mentoring and instructing.
  • May participate on committees or projects, including quality improvement projects.
