About The Position

The Licensed Nursing Assistant works at the Community Hospice House, a facility expanded to meet the need for compassionate end-of-life care in southern New Hampshire. Under the direction of an RN and in collaboration with the care team, the assistant provides hands-on, relationship-driven personal care to help patients remain safe and comfortable. The posted schedule is Monday through Friday, 3:00 p.m. to 11:00 p.m., with weekend and holiday rotation.

Requirements

• Hold a Licensed Nursing Assistant credential in good standing with the New Hampshire Board of Nursing.
• Be CPR certified.
• Pass a satisfactory background check and motor vehicle record review.
• Be reliable, compassionate, and committed to patient-centered care.

Nice To Haves

• Experience in home care.
• Experience caring for patients with dementia, Alzheimer’s disease, COPD, cancer, or post-surgical needs.
• Ability to perform PT/INR checks.

Responsibilities

• Provide personal care, including bathing, dressing, grooming, toileting, and safe transfers.
• Assist with mobility using mechanical devices and proper body mechanics.
• Observe and report changes in patients’ physical or mental condition, intake/output, and overall status.
• Cue patients for medications as directed in the care plan.
• Maintain a clean, safe, and healthy environment in accordance with aseptic and safety standards.
• Communicate professionally with patients, families, and the care team.
• Complete timely and accurate documentation.
• Maintain professional boundaries and provide excellent customer service as a representative of the agency.
