About The Position

The Licensed Nursing Assistant at the Community Hospice House provides compassionate, high-quality personal care to support patient comfort, safety, and well-being. The role works under the direction of an RN and collaborates closely with the care team. The position is hands-on and relationship-driven, helping patients remain safe and comfortable in a hospice setting.

The schedule is Monday through Friday, 3:00 p.m. to 11:00 p.m., with weekend and holiday rotation. The Community Hospice House recently completed an expansion and renovation, adding six private patient suites and increasing its total room count from 10 to 16.

Requirements

• Be a Licensed Nursing Assistant in good standing with the New Hampshire Board of Nursing.
• Be CPR certified.
• Pass a satisfactory background check and motor vehicle record check.
• Be reliable, compassionate, and committed to exceptional patient-centered care.

Nice To Haves

• Experience in home care and/or caring for patients with dementia, Alzheimer’s disease, COPD, cancer, or post-surgical needs.
• Ability to perform PT/INR checks.

Responsibilities

• Provide personal care, including bathing, dressing, grooming, toileting, and safe transfers.
• Use mechanical devices and proper body mechanics to assist patients with safe mobility.
• Observe and report changes in physical or mental condition, intake and output, and overall patient status.
• Cue patients for medications as directed in the care plan.
• Maintain a clean, safe, and healthy environment in accordance with aseptic and safety standards.
• Communicate professionally with patients, families, and the care team.
• Complete timely and accurate documentation.
• Uphold professional boundaries and represent the agency with excellent customer service.
