About The Position

Merakey is seeking a Licensed Therapist to join their team in Sacramento, CA. This role is part of the Intensive Transition Services (ITS) Team and involves providing clinical therapeutic interventions, care coordination, and supportive services to individuals with a mental health diagnosis and Intellectual/Developmental Disability (Dually Diagnosed). Services are delivered in a multi-disciplinary team setting under the clinical supervision of the Program Director.

Requirements

• Master’s degree in Social Work, Psychology, or a related human/social services field.
• Current and unrestricted California license with the Board of Behavioral Sciences as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Clinical Counselor (LPCC).
• Valid driver's license and personal vehicle with insurance.
• Willingness to use your personal vehicle to meet clients at their residences.

Nice To Haves

• At least one (1) year direct service experience with mentally ill individuals
• Experience with individuals with developmental disabilities
• Knowledge of Trauma informed care; experience in CBT, DBT, ACT. Experience working with individuals living with SMI.

Responsibilities

• Provide and collaborate on comprehensive goal plans with individuals and their families.
• Coordinate service activities, including treatment plan development and implementation.
• Serve as a liaison with community agencies and families to maintain treatment coordination.
• Educate and support families, and advocate for individual rights.
• Deliver treatment, rehabilitation, and support services within a team, participate in meetings, and perform necessary cross-training as directed.
• Offer individual support therapy and promote social skill development in community settings.
• Offer practical assistance, advocacy, individualized support, and supervision to help individuals with daily living necessities.
