About The Position

Vanderbilt University Medical Center (VUMC) is seeking a Licensed Clinical Social Worker (LCSW) to join their VPH Social Work - PHP team. This role involves performing behavioral health diagnostic and psycho-social assessments using the DSM-V, implementing therapeutic interventions (group, family, individual, crisis), developing treatment and disposition plans, and maintaining contact with community agencies for referrals. The professional will collaborate with the team to achieve patient care management and system improvement goals, and represent the behavioral health enterprise on committees. The position requires adherence to TN Department of Mental Health, The Joint Commission, and CMS Best Practice Guidelines, providing evidence-based interventions, facilitating transitions to appropriate care levels, and acting as a clinical liaison. Engagement in supervision and upholding ethical standards, confidentiality, and privilege are also key aspects of the role.

Responsibilities

  • Performs behavioral health diagnostic and psycho-social assessments utilizing the DSM-V.
  • Implements group, family and individual psycho therapeutic interventions including crisis intervention.
  • Develops treatment plans and disposition plans.
  • Maintains contact with family and community agencies and makes referrals.
  • Actively collaborates with team to meet patient care management and system improvement goals.
  • Represents the behavioral health enterprise on institutional and community committees/task forces.
  • Provides initial assessment and develops plan of care according to TN Department of Mental Health, The Joint Commission and CMS Best Practice Guidelines and within the scope of their license by the Tennessee Health Related Boards.
  • Provides targeted evidence based interventions into clinical practice with patients.
  • Facilitates transitions to appropriate levels of care via in person, telephone or telehealth contact.
  • Acts as clinical liaison with patient, family, treatment team and community.
  • Contributes to the organization by actively participating in process improvement initiatives and shared decision making.
  • Engages in regularly scheduled supervision as required by the Tennessee Health Related Boards.
  • Upholds ethical standards in accordance with relevant professional association’s code of ethics.
  • Maintains confidentiality and privilege in accordance with TN state codes and statutes.
