Responsibilities
- Provide behavioral health services to older adults in long-term care and rehabilitation centers, including assessment, diagnosis, and treatment planning
- Develop and implement individualized treatment plans to meet the unique needs of each patient
- Conduct ongoing therapy sessions, group therapy, and family therapy
- Collaborate with other healthcare professionals to ensure comprehensive care and coordination
- Maintain accurate and detailed records of patient treatment and progress
- Participate in professional development and continuing education to stay current with industry best practices
