About The Position

CHE Behavioral is seeking a dedicated Licensed Mental Health Professional to provide in-person behavioral health services to residents of skilled nursing facilities in the Harrisburg, Pennsylvania area. The role focuses on supporting older adults through individual and group therapy, assessment, and personalized treatment planning. The clinician works with healthcare professionals and interdisciplinary teams to address each patient’s needs and promote mental health and overall well-being. Requirements

Hold a current license as a Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor (LMHC), or Licensed Marriage and Family Therapist (LMFT).
At least 3 years of experience in mental health services.
Strong communication and interpersonal skills.
Ability to work effectively in a fast-paced environment.
Familiarity with electronic health records and documentation software.

Responsibilities

Provide individual and group therapy sessions to patients in skilled nursing facilities.
Develop and implement personalized treatment plans in collaboration with other healthcare professionals.
Conduct assessments, screenings, and evaluations to identify mental health needs and develop treatment plans.
Collaborate with interdisciplinary teams to ensure comprehensive patient care.
Document patient progress and treatment plans in accordance with industry standards.
Participate in regular consultation and case management meetings with other CHE Behavioral clinicians.
