About The Position

Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Licensed Mental Health Counselor (LMHC) in Fort Plain, New York. This is an outpatient behavioral health role delivering counseling and therapeutic services for individuals and families. It offers flexible scheduling options including hybrid scheduling and four 10-hour shift opportunities within a collaborative interdisciplinary care environment with full clinical and administrative support staff. The organization is mission-driven, focused on long-term patient care and community support, located in Upstate New York.

Requirements

• Clear and active New York LMHC license REQUIRED
• Strong verbal and written communication skills REQUIRED
• Comfortable working with patient populations age 5 and older REQUIRED
• Ability to collaborate within interdisciplinary care environment REQUIRED
• Experience working within electronic health record systems REQUIRED
• Strong patient-centered and team-oriented care approach REQUIRED

Responsibilities

• Deliver individual, family, and group counseling services for patients age 5 and older
• Support patients experiencing behavioral health and substance use concerns
• Create individualized treatment plans in collaboration with medical and clinical teams
• Complete evaluations and implement evidence-based therapeutic approaches
• Maintain timely and accurate documentation within electronic health record systems
• Collaborate with interdisciplinary teams and respond to urgent client needs and crisis situations
• Participate in supervision, training, and continuing education opportunities
