About The Position

Amedisys is hiring a part-time Medical Social Worker to provide home health and hospice social work services. The role is available for 20 or 30 hours on weekdays and requires LMSW or LCSW licensure. The Medical Social Worker supports patients and families with health-related financial, social, and emotional concerns, provides services according to the plan of care, and coordinates with the healthcare team to support safe and effective care. Compensation is $27.50–$32.50; pay may vary by location, job-related knowledge, skills, and experience.

Requirements

• LMSW or LCSW licensure is required.
• Master's or doctorate degree from a school of social work accredited by the Council on Social Work Education.
• One year of social work experience in a healthcare setting.
• Meet state and federal requirements for the position.
• Current CPR certification.

Responsibilities

• Assist patients and families with health-related financial, social, and emotional concerns and help them develop effective coping skills.
• Provide social work services in accordance with the plan of care.
• Communicate changes in a patient's clinical status to other healthcare team members and care center office staff in a timely manner.
• Educate patients, family members, and other team members on medical social work interventions and the treatment plan.
• Perform care planning, transfer, and discharge functions.
• Identify environmental risks or threats to a patient's health, promptly implement appropriate interventions to reduce them, and communicate identified risks to the physician and other healthcare team members.
• Perform other duties as assigned.
