This is a full-time, year-round position based out of Dodge City, Kansas, with travel to surrounding counties. The role involves developing and implementing individualized programs, including case management for children enrolled in Infant Toddler Services. The Social Worker will serve on a multi-disciplinary team, provide comprehensive support to families, assist in identifying and referring to needed services, and maintain accurate records. A key responsibility is coordinating the development and implementation of the Individual Family Service Plan (IFSP) within specified timeframes, including six-month reviews and annual evaluations. The position also involves providing developmental supports using a primary coaching model in the child's natural environment, designing learning activities, and participating in child-find activities. The Social Worker will conduct evaluations and ongoing assessments, serve as a representative on the local interagency coordinating council, and facilitate transition plans to Part B special education programs. Follow-along visits for dismissed or referred children are also required. The role may involve interpreting and making recommendations to the Infant Toddler Services Manager.
