About The Position

This position, based in Syracuse, NY, involves assisting with surveillance and investigation activities related to nursing homes (long-term care). The role is mandated to evaluate facility performance and monitor the quality of care and services provided. The individual will be part of an interdisciplinary survey/investigation team, reviewing allegations of abuse and neglect, performing surveillance at nursing homes and Transitional Care Units (TCUs), and Adult Day Health Care Programs (ADHCP) to ensure compliance with participation requirements. Responsibilities include preparing written reports of findings and Statements of Deficiencies (SOD) within required timeframes, participating in emergency surveys, and State monitoring during Immediate Jeopardy situations.

Requirements

  • Master's degree in Social Work (MSW) from an accredited program
  • Current New York State Licensed Master Social Worker (LMSW) required
  • Minimum two (2) years of clinical experience in a medical facility, preferably a nursing home, required.
  • Ability to obtain Surveyor Minimum Qualifications Test (SMQT) certification within 12 months of employment, required.
  • Ability to travel to an on-site location.
  • Strong interpersonal skills; ability to facilitate positive outcomes in adverse situations; excellent written and verbal communication; superior investigative and analytical skills; ability to conduct interviews effectively; ability to work independently and collaboratively.

Nice To Haves

  • Two (2) years of experience performing nursing home surveillance activities, preferred.

Responsibilities

  • Participate in surveys or complaint investigations, including allegations of abuse and neglect, and perform surveillance activities/tasks at nursing homes (long-term care) to determine compliance with federal and state regulations.
  • Produce written documentation and draft a Statement of Deficiencies (SOD) within the required time frame, using Principles of Documentation.
  • Assess compliance with the Plan of Correction within the required time frame.
  • Complete data entry in federal and state reporting databases as required.
  • Participate in state monitoring during immediate jeopardy situations.
  • Attend meetings as required.
  • Testify in administrative hearings as needed.
  • Adhere to DOH work schedules.
  • Adhere to DOH and IPRO policies and procedures.
  • Travel to onsite locations according to work schedule.
