About The Position
We are seeking a Licensed Master Social Worker (LMSW) to provide comprehensive social work services in a clinical setting. The role involves conducting psychosocial assessments, supporting patients and families, and collaborating with multidisciplinary healthcare teams to ensure high-quality care. This position will cover a full-time equivalent leave of absence.
Requirements
- Master’s Degree in Social Work (MSW) – required
- Licensed Master Social Worker (LMSW) in the State of New York (must obtain within 1 year of hire)
- Strong communication and interpersonal skills
- Knowledge of psychosocial assessment techniques
Nice To Haves
- Experience in a hospital or outpatient medical setting preferred
- Transplant experience is a plus but not required
Responsibilities
- Perform detailed psychosocial assessments for patients
- Develop and implement care plans in coordination with healthcare providers
- Provide emotional support, counseling, and resource guidance to patients and families
- Collaborate with interdisciplinary teams to optimize patient outcomes
- Maintain accurate and timely documentation
