About The Position

Greenlife Healthcare Staffing is seeking a bilingual Spanish Licensed Master Social Worker (LMSW) for a hybrid role in a multi-specialty practice in the Bronx, NY. This position requires at least one year of experience and offers the flexibility of working primarily remotely with one required day on-site per week. The role involves conducting psychosocial assessments, providing supportive counseling, developing and implementing patient care plans, and offering case management services. The LMSW will connect patients with community resources and collaborate with physicians, nurses, and other providers in a multi-specialty setting, ensuring accurate documentation and participating in team meetings for a coordinated care approach.

Requirements

Master of Social Work (MSW) from an accredited program.
Active New York State license as a Licensed Master Social Worker (LMSW) (Required).
Must be residing in the tri-state area.
Minimum of 1 year of relevant experience.
Must be fluent in Spanish.
Proficiency in psychosocial assessments, knowledge of therapeutic modalities, case management, and electronic health records.
Empathetic, culturally competent, excellent communication and collaboration skills, strong organizational abilities, and a commitment to ethical practice.

Responsibilities

Conduct psychosocial assessments and provide supportive counseling under supervision as required for LMSW licensure.
Collaborate with the clinical team to develop and implement patient care plans.
Provide case management services and connect patients with necessary community resources.
Work closely with physicians, nurses, and other providers in a multi-specialty setting.
Maintain accurate and timely documentation of patient interactions and services provided.
Participate in team meetings and contribute to a coordinated care approach.
