About The Position

Ohioans Home Health and Hospice is seeking a passionate Master of Social Worker (MSW) to join our team! Ohioans is looking for an innovative Social Worker who wants to improve the independence and coping skills of home health patients while also assisting in the development of our expanding organization into hospice care. The Master of Social Worker will implement the standards of care for medical social work services and collaborate with primary case managers to assess, plan, facilitate, and advocate options and services to most appropriately meet individual patient's needs. The medical social worker will assist primary case managers to improve patient's overall knowledge and compliance of their care plan.

Requirements

Master's degree accredited by the Council on Social Work Education.
Current license as an Licensed Master Social Worker (LMSW) in Michigan.
Willing to get OH license.
The ability to relate and communicate with a diverse population and groups.
The ability to build and maintain professional helping relationships.
Demonstrates good verbal and written communication.

Nice To Haves

Minimum of one (1) year's experience in health care with an emphasis in either home health or hospice.

Responsibilities
