About The Position
Navitas Healthcare, LLC is seeking a Licensed Master Social Worker (LMSW) for an exciting Travel or Local job in New York, NY. This role involves conducting comprehensive psychosocial assessments, providing supportive counseling, developing discharge plans, and coordinating referrals to community resources. Requirements
- Master’s Degree in Social Work (MSW) required
- Active Licensed Master Social Worker (LMSW) license issued by NYSED required
- Minimum of 1 year of hospital or inpatient social work experience required
- Strong communication, assessment, and case management skills
- Ability to work effectively in a fast-paced hospital environment
- Excellent documentation and organizational abilities
- Compassionate and patient-centered approach to care

- Experience in medical, surgical, or rehabilitation settings preferred
Responsibilities
- Conduct comprehensive psychosocial assessments for hospitalized patients
- Provide supportive counseling and emotional support to patients and families
- Develop and coordinate discharge plans to ensure safe and effective patient transitions
- Arrange appropriate referrals to community resources, rehabilitation services, and support programs
- Collaborate with physicians, nurses, case managers, and interdisciplinary care teams
- Document assessments, interventions, and discharge plans in accordance with hospital standards
- Support patients in coping with illness, hospitalization, and recovery challenges
- Ensure continuity of care through effective coordination and communication
- Maintain compliance with hospital policies and healthcare regulations
