About The Position

As a Master’s Social Worker in the Case Management Department, you will support clients and their families through challenging medical situations. You will help them navigate the emotional, social, and practical implications of medical conditions while coordinating programs and services that improve quality of life. The role works independently under general supervision, with latitude to innovate, and uses clinical judgment and creativity to respond to evolving client needs.

Requirements

• 2–4 years of experience in social work or a related field.
• Strong knowledge of standard social work concepts, practices, and procedures.
• Ability to think critically, act compassionately, and manage multiple priorities.
• Confidence in crisis intervention and family support.
• Master’s Degree in Social Work (MSW) from an accredited School of Social Work.
• Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in South Carolina.
• Basic Life Support (BLS) certification from the American Heart Association (AHA).

Responsibilities

• Interview clients and families to assess social and emotional needs.
• Coordinate and plan individualized programs and activities.
• Provide crisis intervention and emotional support.
• Help families understand the complexities and lifestyle impacts of medical conditions.
• Collaborate with interdisciplinary teams to ensure seamless care.
• Use clinical judgment and creativity to meet evolving client needs.
