Licensed Integrated Behavioral Health Provider Full-Time Roaring Fork Family Practice (RFFP) Clinic
About The Position
We're looking for a Full-Time Licensed Integrated Behavioral Health Provider to join our Behavioral Health team and work at our Roaring Fork Family Practice Clinic located in Carbondale, CO. This role involves planning and coordinating the delivery of quality patient-centered behavioral health care, including assessment, care planning, intervention, consultation, and referral for services where social, economic, emotional, or psychological circumstances present barriers to care or an optimum level of function. The provider will work within their specific behavioral health licensure, provide leadership by cooperating with other clinical personnel, and communicate with physicians on the psychosocial plan of care, all while supporting the philosophy and goals of the clinical area.
Requirements
- DORA Licensure must be as either: Licensed Clinical Social Worker--(LCSW), Marriage and Family Therapist – (LMFT), or Licensed Professional Counselor – LPC.
- Master’s degree in behavioral health Profession.
- Eligible to work in the United States. Visa sponsorship is not available.
Responsibilities
- Plans and coordinates the delivery of quality patient centered behavioral health care, assessment, care, planning, intervention, consultation, and referral for services where the social, economic, emotional or psychological circumstances of the patient presents barriers to care and/or an optimum level of function.
- Works within their specific behavioral health licensure.
- Provides leadership by working cooperatively with other clinical personnel while maintaining standards of professional behavioral health practice.
- Communicates with physician on psychosocial plan of care.
- Supports the philosophy and goals of the individual clinical area.
