About The Position

As a Licensed Field Nurse, you will provide comprehensive, patient-centered care tailored to each client’s individualized service plan. The role focuses on promoting client independence, ensuring safety, and enhancing quality of life for clients and their families. Work involves daily travel to client homes and occasional visits to the agency office, with the work environment varying according to individual client needs. The position is part-time at 20 hours per week, with flexible scheduling and potential weekend and holiday work.

Requirements

Current, valid RN or LPN/LVN license.
Reliable personal vehicle with valid insurance.
Clear criminal background check.
Negative pre-employment drug screening.
Strong verbal and written English communication skills.
Ability to lift at least 25 pounds.

Responsibilities

Provide personal care, including bathing, dressing, grooming, oral care, and hygiene support.
Assist clients with ambulation, wheelchair use, and safe transfers.
Support toileting needs, including catheter and ostomy care.
Reposition bedridden clients and maintain clean, comfortable bedding.
Assist with meal preparation and nutritional intake as needed.
Monitor and record vital signs, including heart rate, blood pressure, respiratory rate, and temperature.
Document intake and output.
Administer and manage medications as prescribed.
Assess client nursing needs on an ongoing basis.
Provide nursing services according to the established care plan.
Promptly report changes in condition, concerns, or progress to the Case Manager and physician.
Complete accurate and timely clinical documentation.
Participate in developing and implementing individualized service plans.
Observe and report changes in clients’ mood, behavior, or emotional status.
Communicate effectively with interdisciplinary team members.
Perform light housekeeping, including laundry, vacuuming, dusting, and kitchen tasks, to maintain a safe and healthy environment.
Organize client living spaces to promote safety and comfort.
Assist with errands and transportation, including medication pickup and grocery shopping.
Practice within the legal scope of licensure and maintain professional appearance and boundaries.
Promote teamwork, unity, and a culture of respect.
Adapt to additional responsibilities as assigned by leadership.
