About The Position

As a Licensed Field Nurse, you will provide comprehensive, patient-centered care tailored to each client’s individualized service plan. You will help promote independence, ensure safety, and enhance quality of life for clients and their families. The position involves providing care in client homes, collaborating on care plans, documenting clinical information, and performing supportive services. Daily travel to client homes and occasional visits to the agency office are required. Scheduling is flexible and may include weekends and holidays, with compensation provided for those shifts.

Requirements

Current, valid RN or LPN/LVN license.
Reliable personal vehicle with valid insurance.
Clear criminal background check.
Negative pre-employment drug screening.
Strong verbal and written English communication skills.
Ability to lift at least 25 pounds.

Responsibilities

Provide personal care, including bathing, dressing, grooming, oral care, and hygiene support.
Assist clients with mobility, ambulation, wheelchair use, and safe transfers.
Support toileting needs, including catheter and ostomy care.
Reposition bedridden clients and maintain clean, comfortable bedding.
Assist with meal preparation and nutritional intake as needed.
Monitor and record vital signs, including heart rate, blood pressure, respiratory rate, and temperature.
Document client intake and output.
Administer and manage medications as prescribed.
Assess client nursing needs on an ongoing basis and provide services in accordance with the established care plan.
Promptly report changes in condition, concerns, or progress to the Case Manager and physician.
Complete accurate and timely clinical documentation.
Participate in the development and implementation of individualized service plans.
Observe and report changes in clients’ mood, behavior, or emotional status.
Communicate effectively with interdisciplinary team members.
Perform light housekeeping, including laundry, vacuuming, dusting, and kitchen tasks, to maintain a safe and healthy environment.
Organize client living spaces to promote safety and comfort.
Assist with errands and transportation, including medication pickup and grocery shopping.
Practice within the legal scope of licensure and maintain professional appearance and boundaries.
Promote teamwork, unity, and a culture of respect.
Adapt to additional responsibilities as assigned by leadership.
