High Focus Centers in Branchburg is seeking a compassionate and dedicated Licensed Counselor to join our team of behavioral healthcare professionals providing Partial Hospitalization (PHP) and Intensive Outpatient (IOP) services. Requirements
- Active New Jersey licensure as an LAC, LSW, LPC, or LCSW.
- Compassion, professionalism, strong clinical skills, and a genuine commitment to helping individuals and families navigate mental health and substance use challenges.
Responsibilities
- Conduct comprehensive assessments of clients' behavioral, emotional, cognitive, spiritual, and social needs.
- Develop individualized treatment plans in collaboration with clients, families, and the multidisciplinary treatment team.
- Facilitate individual, family, and group therapy sessions within PHP and IOP programming.
- Conduct Level of Care Assessments as required.
- Monitor client progress and collaborate with treatment team members to support positive treatment outcomes.
- Maintain timely, accurate, and confidential clinical documentation.
- Participate in utilization review, aftercare planning, and outcome measurement processes.
- Communicate with referral sources and outpatient providers as appropriate to promote continuity of care.
- Contribute to curriculum development, clinical training, and ongoing program improvement.
- Complete required organizational and accreditation-related training.
