About The Position

Diversus Health is seeking a Licensed Counselor to join our Assertive Community Treatment (ACT) Team, providing intensive, community-based mental health support for adults with Serious and Persistent Mental Illness (SPMI). Requirements

Active, unrestricted Colorado licensure as LCSW, LPC, or LMFT
Experience with SPMI populations, especially psychosis or co-occurring conditions
Comfort providing care in community or non-traditional settings
Strong clinical judgment, communication skills, and teamwork orientation
Valid Colorado driver’s license (for community visits)
Master’s degree in one of the behavioral sciences or related fields from an accredited college or university
Clinical licensure (LMFT, LCSW, LPC, PsyD etc.)
Required experience with completing DAs, treatment plans and clinical case notes
Effective written and verbal communication skills
Ability to demonstrate and model stable, appropriate boundaries with clients
Ability to complete and submit documentation of services and other documents in a timely manner, ideally by end of end or within 24 hours; concurrent/collaborative documentation is a plus
Comfort and familiarity working with a diverse high needs client base
Proficient in the use of Office 365 and Electronic Health Record systems

Responsibilities

Conduct clinical assessments and create personalized treatment plans
Provide direct therapy and crisis intervention in community-based settings
Collaborate with psychiatry, nursing, care coordinators, and community partners
Serve as a trusted liaison for clients navigating hospitals, outpatient services, and housing resources
Document care accurately in our EHR while maintaining compliance and confidentiality
Participate in team meetings, case reviews, and professional development
Evaluate mental health diagnosis, create, and implement a treatment plan, complete ongoing documentation including further diagnosis, treatment plan reviews, and case notes according to company policy
Provide excellent customer service for clients and collaborate with a dynamic team to further the mission of filling gaps in our community
Utilize creativity in interventions to help clients achieve and exceed goals
Prepare and submit individual documentation for each session per company guidelines and protocol
For Full-Time status clinicians must maintain a minimum of 30 client visits per week
Coordinate services with case managers, families, work personnel, medical personnel, other Diversus Health staff, and school staff as needed
Attend and participate in all clinical staff meetings and trainings
