About The Position

Diversus Health is seeking a Licensed Counselor to join our Assertive Community Treatment (ACT) Team, providing intensive, community-based mental health support for adults with Serious and Persistent Mental Illness (SPMI). Requirements

  • Active, unrestricted Colorado licensure as LCSW, LPC, or LMFT
  • Experience with SPMI populations, especially psychosis or co-occurring conditions
  • Comfort providing care in community or non-traditional settings
  • Strong clinical judgment, communication skills, and teamwork orientation
  • Valid Colorado driver’s license (for community visits)
  • Master’s degree in one of the behavioral sciences or related fields from an accredited college or university
  • Clinical licensure (LMFT, LCSW, LPC, PsyD etc.)
  • Required experience with completing DAs, treatment plans and clinical case notes
  • Effective written and verbal communication skills
  • Ability to demonstrate and model stable, appropriate boundaries with clients
  • Ability to complete and submit documentation of services and other documents in a timely manner, ideally by end of end or within 24 hours; concurrent/collaborative documentation is a plus
  • Comfort and familiarity working with a diverse high needs client base
  • Proficient in the use of Office 365 and Electronic Health Record systems

Responsibilities

  • Conduct clinical assessments and create personalized treatment plans
  • Provide direct therapy and crisis intervention in community-based settings
  • Collaborate with psychiatry, nursing, care coordinators, and community partners
  • Serve as a trusted liaison for clients navigating hospitals, outpatient services, and housing resources
  • Document care accurately in our EHR while maintaining compliance and confidentiality
  • Participate in team meetings, case reviews, and professional development
  • Evaluate mental health diagnosis, create, and implement a treatment plan, complete ongoing documentation including further diagnosis, treatment plan reviews, and case notes according to company policy
  • Provide excellent customer service for clients and collaborate with a dynamic team to further the mission of filling gaps in our community
  • Utilize creativity in interventions to help clients achieve and exceed goals
  • Prepare and submit individual documentation for each session per company guidelines and protocol
  • For Full-Time status clinicians must maintain a minimum of 30 client visits per week
  • Coordinate services with case managers, families, work personnel, medical personnel, other Diversus Health staff, and school staff as needed
  • Attend and participate in all clinical staff meetings and trainings
